Tournament participants are also required to submit the following prior to the tournament:

1. Team Roster
   
   • You may fill this out online using our roster submit form
     
2. Coaches Conduct Form
   
   • You may fax or mail the coaches conduct form, please ensure all signatures are on the completed form
     
3. Birth Certificates for each player
   
   
4. Insurance Certificate listing Diamonds In The Rough Park, LAH & Associates as the certificate holder.
